How updates should be handled
When a report changes, the update should say what changed, when it changed, and why. Corrections should be ordinary, visible, and non-defensive.
Plainly: if a fact changes, fix it. If a claim was too strong, narrow it. If a source is uncertain, label it. If a report is superseded, say so.
